You should receive the following paperwork upon completion of your online registration for summer camp. These forms must be completed every year. Campers will not be allowed to stay at camp without all paperwork on file. Only those who are attending Family Camp do not need to complete these forms.

2017 Signature Form
Obtain doctor's signature, sign, and return prior to June 1 along with a copy of the camper's health insurance card and the policy holder's driver's license.

Sign and return prior to June 1.

If you are uncertain if your paperwork has been submitted and need to check, here's how:

  • Log into your account.
  • Scroll down to the "Forms" section on the right-hand side of the screen. The "Online Forms" tab should be highlighted.
  • Click "Open Camper Profile Sheet."
  • Be sure all boxes are checked (Physician Signature, Parent Signature, Drivers's License, Insurance, Community Covenant).
  • If anything is unchecked, return to this page and download the needed forms.
